Certificate Order Form

Please complete the following order form for your certificate. The information you enter in each field will be printed on your certificate, so please be sure there are no errors. M.D. and M.B.B.S. degrees are not interchangeable. Abbreviations for your memberships in specialty societies will not be included on the certificate.

Diplomates who have recently passed the 2006 exams, please complete and submit this form no later than January 31, 2007. If we do not receive a certificate order form by this date, we will print your name as it appears on your application and you will be financially responsible for any corrections.
